1. NEW E-PASSPORT REQUIREMENTS (FRESH APPLICATION)

Fresh E-Passport Application Requirements for Adults

1. Print out of the Completed online passport application form (Please check all fields for any errors).
2. Acknowledgement Slip and Passport Payment slip.
3. Two (2) recent passport size photographs.
4. Copy of Birth Certificate (original must be brought for sighting).
5. Local Government Letter of identification/Origin from the State of origin.
6. 28 Euro postal order from An Post Ireland made payable to the “Embassy of Nigeria in Ireland".
7. National Identification Number (NIN) Slip or evidence of Enrollment.
8. Original and Photocopy of the (OLD) Nigerian MRP Passport data page. (Please include photocopy of ANY OBSERVATION Page).
9. Proof of address (Such as Utility Bill, etc).
10. Guarantor's form (Duly SIGNED & STAMPED by a Solicitor/Notary/Commissioner for OATHS).
11. Photocopy of the Guarantor's e-passport data page, (NOTE: Guarantor must be a Nigerian Citizen).
12. One Passport Photograph of Guarantor.

NOTE: Fresh e-passport applications are only accepted from persons born in Ireland or elsewhere outside Nigeria or Applicants that holds the old MRP Passport.

Fresh E-Passport Application Requirements for Children / Minors (under the age of 18)

1. Print out of the Completed online passport application form (Please check all fields for any errors).
2. Acknowledgement Slip and Passport Payment slip.
3. Two (2) recent passport size photographs.
4. Copy of Birth Certificate (original must be brought for sighting).
5. National Identification Number (NIN) Slip or evidence of Enrollment.
6. 28 Euro postal order from An Post Ireland made payable to the “Embassy of Nigeria in Ireland".
7. A letter of consent duly signed by the Father and Mother/Legal Guardian.
8. Photocopy of the passport data page of the father and mother.
9. Passport photographs of both parents.
10. If the mother has sole custody, a copy of the court order granting her full custody or authority to waive the rights of the father.
11. In case of a Legal Guardian, photocopy of Court order awarding custody and the Passport data page of the Legal Guardian.
12. Proof of address (Such as Utility Bill, etc.).

3. Two (2) recent passport size photographs.
4. Original and Photocopy of the current e-passport.
5. National Identification Number (NIN) Slip or evidence of Enrollment.
6. 28 Euro postal order from An Post Ireland made payable to the “Embassy of Nigeria in Ireland".
7. Proof of address (Such as Utility Bill, etc).

3. CORRECTION OF DATA DUE TO CHANGE IN MARITAL STATUS

E-Passport Application Requirements for Change of surname due to Marriage

1. Written application of request for Change of Surname due to Marriage.
2. Print out of the Completed online passport application form (Please check all fields for any errors).
3. Acknowledgement Slip and Passport Payment slip.
4. Two (2) recent passport size photographs.
5. Original and Photocopy of the current e-passport.
6. 28 Euro postal order from An Post Ireland made payable to the “Embassy of Nigeria in Ireland".
7. Copy of Marriage Certificate or Declaration of Marriage (Original copy to be brought for sighting).
8. Photocopy of the data page of Husband's passport.
9. Affidavit for Change of Name from a Magistrate or High Court deposed to by the Applicant (if the marriage was done in Nigeria).
10. Copy of Newspaper Publication of Change of Name (21 days after publication) (if the marriage was done in Nigeria).
11. Statutory evidence of change of name e.g. deed poll (if the marriage was done elsewhere outside Nigeria in lieu of 9 and 10).
12. Proof of address (Such as Utility Bill, etc).

E-Passport Application Requirements for Change of surname due to Divorce

1. Written application of request for Change of Surname due to Divorce.
2. Print out of the Completed online passport application form (Please check all fields for any errors).
3. Acknowledgement Slip and Passport Payment slip.
4. Two (2) recent passport size photographs.
5. Original and Photocopy of the current e-passport.
6. 28 Euro postal order from An Post Ireland made payable to the “Embassy of Nigeria in Ireland".
7. Evidence of Maiden Name.
8. In case of Re-marriage, Certificate of Marriage with Current Spouse.
9. Affidavit for Change of Name from a Magistrate or High Court deposed to by the Applicant (if the Divorce was done in Nigeria).
10. Copy of Newspaper Publication for Change of Name (21 days after publication) (if the Divorce was done in Nigeria).
11. Statutory evidence of change of name e.g Deed Poll (if the Divorce was done elsewhere outside Nigeria in lieu of 9 and 10).
12. Proof of address (Such as Utility Bill, etc).

E-Passport Application Requirements for Change of surname due to Death of Husband

1. Written application of request for Change of Surname due to Death of Husband.
2. Print out of the Completed online passport application form (Please check all fields for any errors).
3. Acknowledgement Slip and Passport Payment slip.
4. Two (2) recent passport size photographs.
5. Original and Photocopy of the current e-passport.
6. 28 Euro postal order from An Post Ireland made payable to the “Embassy of Nigeria in Ireland".
7. Certificate of Death of spouse from National Population Commission (NPC) or any competent Authority if the death occurred outside Nigeria.
8. Evidence of Maiden Name.
9. In case of Re-marriage, Certificate/Notification of Marriage with Current Spouse.
10. Affidavit for Change of Name from a Magistrate or High Court deposed to by the Applicant (if the subsequent marriage was done in Nigeria).
11. Copy of Newspaper Publication for Change of Name (21 days after publication) (if the subsequent marriage was done in Nigeria).
12. Statutory evidence of change of name e.g. Deed Poll (if the subsequent marriage was done elsewhere outside Nigeria in lieu of 9 and 10).
13. Proof of address (Such as Utility Bill, etc).
