REQUIREMENTS FOR CHANGE OF NAME
A written application for change of name is to be submitted with the following documents:
- Embassy ID
- Birth Certificate
- Marriage or Divorce Certificate (whichever is applicable)
- Affidavit of Change of Name
- Newspaper publication of change of name in a National Newspaper in Nigeria
- Photocopy of data page of International Passport
- Recent Utility Bill as proof of address (not exceeding previous 3 months)
- Two (2) passport sized photographs
