Application Requirements for fresh E-passport for married women
Printed Embassy Online confirmation of appointment slips (NGA04).
Complete SIGNED Online Application Form (Please check all fields for any errors).
Consent letter from the husband.
Application letter duly signed by the Applicant addressed to the Passport Officer requesting for a change in details due to Marriage
Two (2) recent passport photos of the applicant.
Photocopy of applicant's Birth Certificate or Age Declaration.
Local government of origin certificate.
Photocopy of a Marriage Certificate issued by an authorised and recognised Body under the Law showing new names (Original copy to be sighted)
Photocopy of the data page of husband's passport
Guarantor's form {Duly SIGNED & STAMPED by a Notary Lawyer/Commissioner of OATHS}.
Photocopy of the Guarantor's passport data page, National ID Card or Driver's licence.
One passport photo of Guarantor.
Proof of address (Photocopy of a Utility Bill, e.t.c.).
Photocopy of GNIB Card.
E-passport Payment slip.
E-passport Acknowledgement Slip.
NOTE: If marriage was contracted in Nigeria, the following also applies:
Photocopy of a Sworn Court Affidavit (original copy to be sighted) and publication from a recognised National Nigerian Newspaper showing the change in status.
Requirements for extension of validity of E-passport:
Personal request from the applicant stating his/her reason for the extension of validity of his/her passport
Evidence of need of extension e.g. Letter from his/her school and admission letter for students or visa page and data page of the passport in the case of non-students.
In case of LOST Passports, Applicants MUST produce:
Personal request from the applicant stating how his/her E-passport got lost
Photocopy of a duly stamped and endorsed Police Report (Bring Original for sighting) and court affidavit for the lost E-passport obtained from the applicant's country of residence
Data page of the E-passport(s) of the said lost passport(s)
Application Requirements for E-passport in case of change of name due to divorce:In addition to the requirements of marriage above, there is the need to bring the following evidence of marriage dissolution
Application letter duly signed by the Applicant addressed to the Passport Officer requesting a change in details due to divorce
Where divorce takes place in Nigeria, Original copy of Newspaper publication (A recognized Nigerian National Newspaper; showing the changes in status) must be brought for sighting.
Photocopy of the Decree Nisi (original copy to be sighted)
Photocopy of the Decree Absolute dissolution of marriage (original copy to be sighted)
If it is abroad, the only requirement is to forward the Authentic court ruling by both respondents on the dissolution of their marriage
Application Letter duly signed by the Applicant addressed to the Passport Officer requesting for a Change in Details due to Marriage.
Photocopy of a Marriage Certificate issued by an authorised and recognised Body under the Law showing new names (Original copy to be sighted).
If the marriage was contracted in Nigeria, photocopy of a Court Affidavit (original copy to be sighted) and Original copy of Newspaper publication from a recognized Nigerian National Newspaper; showing the changes in status must be brought for sighting.
Photocopy of applicant's Birth Certificate or Age Declaration.
Local Government of origin certificate.
Photocopy of the (old) Nigerian MRP Passport data page.
(Please include photocopy of ANY OBSERVATION Page.)
Guarantor's form {Duly SIGNED & STAMPED by a Notary Lawyer/Commissioner of OATHS}.
Photocopy of the Guarantor's passport data page, National ID Card or Driver's licence.
One passport photo of Guarantor.

Photocopy of the Decree Nissi (original copy to be sighted).
Photocopy of the Decree Absolute (original copy to be sighted).
If the divorce took place in Nigeria, Original copy of Newspaper publication (A recognized Nigerian National Newspaper; showing the changes in status) must be brought for sighting.
Application Letter duly signed by the Applicant addressed to the Passport Officer requesting for a Change in Details due to spelling mistakes/errors in passport data.
The Applicant must state clearly the changes to be made on the e-passport.
The application must be supported with documents which include but are not limited to:
Previous passport showing correct details.
Birth certificate.
Any other documents evidencing correct data name.
