Change of Name

REQUIREMENTS FOR CHANGE OF NAME

A written application for change of name is to be submitted with the following documents:

1. Embassy ID
2. Birth Certificate
3. Marriage or Divorce Certificate (whichever is applicable)
4. Affidavit of Change of Name
5. Newspaper publication of change of name in a National Newspaper in Nigeria
6. Photocopy of data page of International Passport
7. Recent Utility Bill as proof of address (not exceeding previous 3 months)
8. Two (2) passport sized photographs
