1. New E-Passport Requirements

Adults

E-Passport Application Requirements for Adults
  1. Printed Embassy Online confirmation of appointment slips (NGA04).
  2. Complete SIGNED Online Application Form (Please check all fields for any errors).
  3. Two (2) recent passport photos of the applicant.
  4. Photocopy of applicant's Birth Certificate or Age Declaration.
  5. Local government of origin certificate.
  6. Photocopy of the (old) Nigerian MRP Passport data page.
    • (Please include photocopy of ANY OBSERVATION Page.)
  7. Guarantor's form {Duly SIGNED & STAMPED by a Notary Lawyer/Commissioner of OATHS}.
    • Photocopy of the Guarantor's passport data page, National ID Card or Driver's licence.
    • One passport photo of Guarantor.
  8. Proof of address (Photocopy of a Utility Bill, e.t.c.).
  9. Photocopy of GNIB Card.
  10. E-passport Payment slip.
  11. E-passport Acknowledgement Slip.

Children/Minors

E-Passport Application Requirements for Children / Minors
  1. Printed Embassy Online confirmation of appointment slip (NGA04).
  2. Complete SIGNED Online Application Form (Please check all fields for any errors).
  3. Two (2) recent passport photos of the applicant.
    • One (1) recent passport photo of father and mother
  4. Photocopy of Birth Certificate.
  5. Photocopy of the (old) Nigerian MRP Passport data page (if any).
  6. A letter of consent DULY signed by the father and mother / Legal Guardian.
    • Photocopy of the passport data page of the father and mother.
  7. If the mother has sole custody, she must provide a copy of the court order granting her full custody or authority to waive the rights of the father
  8. In case of a Legal Guardian, photocopy of Court order awarding custody plus the Passport data page.
  9. Photocopy of parent's / legal guardian's GNIB Card.
  10. Proof of Parents / Legal Guardian's address (Photocopy of a Utility Bill, etc.)
  11. E-Passport Payment slip and E-passport Acknowledgement Slip.

Married Women

Application Requirements for fresh E-passport for married women
  1. Printed Embassy Online confirmation of appointment slips (NGA04).
  2. Complete SIGNED Online Application Form (Please check all fields for any errors).
  3. Consent letter from the husband.
  4. Application letter duly signed by the Applicant addressed to the Passport Officer requesting for a change in details due to Marriage
  5. Two (2) recent passport photos of the applicant.
  6. Photocopy of applicant's Birth Certificate or Age Declaration.
  7. Local government of origin certificate.
  8. Photocopy of a Marriage Certificate issued by an authorised and recognised Body under the Law showing new names (Original copy to be sighted)
  9. Photocopy of the data page of husband's passport
  10. Guarantor's form {Duly SIGNED & STAMPED by a Notary Lawyer/Commissioner of OATHS}.
    • Photocopy of the Guarantor's passport data page, National ID Card or Driver's licence.
    • One passport photo of Guarantor.
  11. Proof of address (Photocopy of a Utility Bill, e.t.c.).
  12. Photocopy of GNIB Card.
  13. E-passport Payment slip.
  14. E-passport Acknowledgement Slip.
NOTE: If marriage was contracted in Nigeria, the following also applies:
  1. Photocopy of a Sworn Court Affidavit (original copy to be sighted) and publication from a recognised National Nigerian Newspaper showing the change in status.

E-passport Validity Extension

Requirements for extension of validity of E-passport:
  1. Personal request from the applicant stating his/her reason for the extension of validity of his/her passport
  2. Evidence of need of extension e.g. Letter from his/her school and admission letter for students or visa page and data page of the passport in the case of non-students.

Lost Passports

In case of LOST Passports, Applicants MUST produce:
  1. Personal request from the applicant stating how his/her E-passport got lost
  2. Photocopy of a duly stamped and endorsed Police Report (Bring Original for sighting) and court affidavit for the lost E-passport obtained from the applicant's country of residence
  3. Data page of the E-passport(s) of the said lost passport(s)
  4. An interview may be required.

Change of Name: Due to Divorce

Application Requirements for E-passport in case of change of name due to divorce: In addition to the requirements of marriage above, there is the need to bring the following evidence of marriage dissolution
  1. Application letter duly signed by the Applicant addressed to the Passport Officer requesting a change in details due to divorce
  2. Where divorce takes place in Nigeria, Original copy of Newspaper publication (A recognized Nigerian National Newspaper; showing the changes in status) must be brought for sighting.
    • Photocopy of the Decree Nisi (original copy to be sighted)
    • Photocopy of the Decree Absolute dissolution of marriage (original copy to be sighted)
  3. If it is abroad, the only requirement is to forward the Authentic court ruling by both respondents on the dissolution of their marriage

Change of Name: Due to Family Resolution

Application Requirements for E-passport in case of change of name due to family resolution:
  1. Personal request from the applicant stating why the family resolution to change their name.
  2. Evidence of deed poll of change of name due to family resolution obtained from the country of residence.
  3. Data page of their E-passport(s).
  4. Photocopy of applicant's Birth Certificate or Age Declaration.
  5. Any other supporting documents to butress his/her claims.

Change of Name: Due to Criminal Intent

Change of data due to Criminal Intent
  • It is punishable under the Passport Miscellaneous Offences Act and Penalty in Immigration Act of 2015 as amended, Section 10, Sub Section A to G

2. E-Passport Renewal Requirements

Notice to Applicants Willing to Renew His/her E-Passport

All applicants wishing to renew their e-passports are required to present the following to the Embassy:
  • Embassy Online confirmation of appointment slips (NGA04).
  • Printed and duly completed online e-passport application forms.
  • Printed payment and acknowledgement slips.
  • Photocopy of the data page of their current e-passport.
  • 2 passport sized photos. 
  • Photocopy of a Utility Bill (as proof of address).

3. Change of Details

Due to Marriage

(The requirements are as follows):
  1. Application Letter duly signed by the Applicant addressed to the Passport Officer requesting for a Change in Details due to Marriage.
  2. Photocopy of a Marriage Certificate issued by an authorised and recognised Body under the Law showing new names (Original copy to be sighted).
  3. If the marriage was contracted in Nigeria, photocopy of a Court Affidavit (original copy to be sighted) and Original copy of Newspaper publication from a recognized Nigerian National Newspaper; showing the changes in status must be brought for sighting.
  4. Photocopy of applicant's Birth Certificate or Age Declaration.
  5. Local Government of origin certificate.
  6. Photocopy of the (old) Nigerian MRP Passport data page.
    • (Please include photocopy of ANY OBSERVATION Page.)
  7. Guarantor's form {Duly SIGNED & STAMPED by a Notary Lawyer/Commissioner of OATHS}.

Due to Divorce

(The requirements are as follows):
  1. Application Letter duly signed by the Applicant addressed to the Passport Officer requesting for a Change in Details due to Divorce.
  2. Photocopy of the Decree Nissi (original copy to be sighted).
  3. Photocopy of the Decree Absolute (original copy to be sighted).
  4. If the divorce took place in Nigeria, Original copy of Newspaper publication (A recognized Nigerian National Newspaper; showing the changes in status) must be brought for sighting.

Due to Errors

(The requirements are as follows:):
  1. Application Letter duly signed by the Applicant addressed to the Passport Officer requesting for a Change in Details due to spelling mistakes/errors in passport data.
  2. The Applicant must state clearly the changes to be made on the e-passport.
  3. The application must be supported with documents which include but are not limited to:
    • Previous passport showing correct details.
    • Birth certificate.
    • Any other documents evidencing correct data name.
      Note: All the original documents must be brought for sighting.
